What the research says about isometrics and exercise, graded by how sure it is. None of it tests WITH itself: no study has looked at random 15-second holds spread through a day.
Strong: consistent across large reviews or meta-analyses.
Moderate: good evidence with limits, such as fewer studies or a different type of exercise.

Strong
Training
- Isometric training builds strength.
- Isometric strength gains show up mostly on isometric tests. On moving strength tests, isometrics tie with lifting.
- Isometric training lowers resting blood pressure, and ranked first of all exercise types for it: about 8/4 mmHg lower, across 270 trials and 15,827 people. The protocol: 4 × 2 minutes at about 30% effort, 3 times a week, for at least 4 to 8 weeks. The wall squat was the best single exercise for lowering systolic (top number) pressure.
- Holding gives out sooner than pushing at the same load, except in trunk muscles: in older adults at moderate effort, 10.6 minutes against 22.8. Heart rate and blood pressure are the same for both.
- Tendons only adapt to hard efforts, 70% of max or more. How hard matters, not the type of contraction.
- Tendons adapt more over programmes longer than 12 weeks than over shorter ones.
- Light loads (60% of max or less) build as much muscle as heavy loads when sets go to failure. Max strength rises more with heavy loads; isometric strength rises equally. 21 studies.
Mind
- A single bout of exercise lifts mood and lowers anxiety straight afterwards. 103 studies, 4,671 people. Exercise in general.
- Regular physical activity reduces depression, anxiety and psychological distress, with medium effects. 1,039 trials, 128,119 people. All types, strength included.
- Strength training reduces depressive symptoms. 33 trials, 1,877 people. Smaller effect in the best-controlled trials.
- A single bout of exercise gives a small boost to thinking and reaction time. 113 studies, young adults. Exercise in general.
- Regular exercise improves thinking in people over 50. 36 studies, strength included.
- Regular exercise raises energy and lowers fatigue. 81 trials, 7,050 people. Exercise in general.
- Quality note: the mood meta-analyses are built mostly on low-quality trials. The direction is consistent across all of them, but the exact effect sizes are soft.
Body
- Exercise reduces chronic lower back pain by about 15 points on a 100-point scale compared with no treatment, a clinically meaningful difference. 249 trials, moderate certainty. All exercise types.
- Strength training reduces body fat (about 1.5 percentage points) and visceral fat in healthy adults. Small effect. 58 studies.
Moderate
Training
- Isometric training can build muscle.
- For muscle growth, how hard you push matters little.
- Total volume matters more than effort. Longer holds, more total hold time or longer muscle lengths improve results more than pushing harder.
- Explosive contractions build faster force. For muscle and max strength, explosive vs slow build makes little difference.
- Holding at longer muscle lengths builds more muscle (a small effect) and carries over better to movement. Strength gains concentrate at the angle you trained.
- Breaking up sitting with 3 minutes of simple exercise every 30 minutes lowers blood sugar, insulin and triglycerides after meals, and lowers resting blood pressure. Shown in one-day studies of people with type 2 diabetes, using partly moving exercises.
- Exercise snacks (vigorous bursts under a minute) improve fitness and cholesterol, with no change in body fat. These were cardio bursts, not isometrics.
- Holding needs more complex nerve control than pushing, and pushing reaches higher peak force.
- Straining against a closed throat (the Valsalva manoeuvre) drives blood pressure to extreme levels: 311/284 mmHg at a max leg press, against 198/175 breathing out slowly. Individual peaks went above 480/350.
- The blood-pressure rise tracks how hard you try, not how much force you make. Isometric and moving contractions at the same effort raise it equally.
- Above roughly 80% of max, a brief breath-hold happens on its own.
- Breath-holding raises pressure inside the abdomen, which is thought to stabilise the spine. The cost is the blood-pressure spike.
- Isometric contractions cause little muscle damage compared with lowering (eccentric) movements.
- In regular strength training, how often you train doesn't change muscle growth when weekly volume is equal. Training more often helps only by adding volume. These studies used moving exercise, not isometrics.
- Training fasted or fed makes no difference to muscle, strength or lean mass over time.
- Eating first improves performance mainly in sessions longer than 45 minutes after a fast of 8 hours or more.
- Exercise after a meal lowers blood sugar more than exercise before it. Exercise before the meal was no better than none.
- Warming up a muscle improves fast force, not max force.
- Structured warm-up programmes cut leg injuries by 30 to 50% in sports. These were running, jumping and cutting sports, not isometric holds.
- Cool-downs don't prevent injury or soreness, and don't help next-day performance.
- Strength is higher in the late afternoon and evening than in the morning.
- Band training with moving reps builds strength and function in older adults, but isn't better than other resistance training.
- Rep speed barely matters for muscle: reps lasting 0.5 to 8 seconds grew muscle about equally. Very slow reps (over 10 seconds) may work less well, but that's thinly studied. 8 studies.
- Training one side strengthens the other side by about 12% (9% arms, 16% legs), through the nervous system. General training, not isometric-specific.
Mind
- Strength training reduces anxiety symptoms, more in healthy people. 16 trials, 922 people.
- A single strength session improves self-control and mental flexibility straight afterwards. 12 trials.
- A single bout of exercise raises feelings of energy. 16 experiments, 678 people. Fatigue doesn't reliably drop.
- Regular strength training improves sleep quality. 13 studies.
- Exercise raises BDNF in the blood, a protein thought to support learning and mood. Strength training raised it in older adults. Mostly aerobic studies.
- Exercise raises endocannabinoids, the body's own cannabis-like chemicals linked to the "runner's high". Mostly running and cycling.
- Short breaks of 10 minutes or less raise energy and lower fatigue, but don't improve work performance. 22 samples, 2,335 people.
Body
- Exercise combined with education cuts the risk of a new back-pain episode by about 45% over the next year. The protection fades after a year unless exercise continues. 21 trials, 30,850 people.
- Neck and shoulder strengthening reduces neck pain in office workers who already have it, and did better than general fitness training. 29 trials.
- Exercise improves physical function in knee arthritis. Pain relief is less certain. 139 trials, 12,468 people. Mostly strengthening.
- Long sitting impairs how well the blood vessels in the legs work. 22 trials.
- Breaking up sitting with short activity bouts restores leg blood-vessel function and blood flow. Large muscles moving for 2 to 5 minutes every 30 to 60 minutes worked best. 36 studies, 632 people. Mostly walking and stairs.
- Exercise improves body image. Small effect. 57 studies.
Limited
Training
- Longer holds (20 to 30 seconds) build more muscle and tendon than very short ones (1 to 3 seconds), likely because sustained holds restrict blood flow. Two small studies.
- In one 12-week trial, 3-second slow-build holds grew more muscle (8.1% vs 2.6%) and more max strength (23% vs 17%) than 1-second explosive ones. The slow holds were also longer, which may explain the gap.
- For max strength, reviews recommend 1 to 5-second holds at 80 to 100% effort, at several angles. A recommendation, not a tested comparison.
- Contracting hard with no load can build muscle, but builds less strength. The key study used moving contractions, not static ones. A study of 30-second no-load squeezes between sets found a possible small gain in thigh size and possibly smaller leg-strength gains, both uncertain.
- Isometrics at a long muscle length grow muscle about as well as full-range lifting. One 2025 study, whose authors call the result uncertain.
- Brief daily maximal holds built strength in older studies of 5 to 12 people each, by 10% to 92%, with no muscle growth wherever it was measured. The first, Hettinger and Müller in 1953, reported about 5% a week from one 6-second hold a day. A modern test of a single 3-second hold a day found no isometric strength gain and no muscle growth.
- Whether effort matters for strength: reviews disagree.
- Holding your breath during a hold raises blood pressure further, even without straining.
- Holding your breath gives no extra force. Breathing out forcefully gave the same or more.
- Daily isometric training has been used in studies, at 5 to 7 days a week. No study was found that tracks harm from it.
- Explosive and slow-build training stiffened the kneecap tendon about equally. One 12-week trial.
- Explosive contractions may be painful or unsuitable for people in rehab.
- Isometric holds (5 × 45 seconds at 70 to 80%) gave immediate pain relief for kneecap tendon pain. One study.
- Exercise starting about 30 minutes after a meal may work best for blood sugar in type 2 diabetes.
- Resting 3 or 10 seconds between contractions made no meaningful difference to tendons. One study.
- Imagined contractions alone, 15 minutes a day for 12 weeks, raised elbow strength 13.5% and finger strength 35%, with no training. Groups of 8.
- Focusing on the working muscle raised its activation during push-ups. For the triceps, it depended on training experience.
- Focusing on the muscle grew the arms more (12.4% vs 6.9%); no difference for the legs. 30 untrained men, 8 weeks.
- Knee extensions in the stretched range grew the quads more than in the shortened range.
- Triceps trained overhead (stretched) grew substantially more than with the arm at the side.
- Hip thrusts (hardest at short length) grew the glutes about as much as squats. 9 weeks, untrained.
- Long-length holds stiffened the tendon and raised strength at every angle. Short-length holds raised strength only near the trained angle, with no tendon change. Muscle growth was similar. 9 men, 12 weeks.
Mind
- Breaking up sitting improves thinking: the evidence certainty is rated low.
Body
- Strength training for tension headaches: the only evidence is rated low quality, and two trials conflict.
- Exercise and self-esteem: the meta-analyses are in young people only, not adults.
- Exercise alone preventing back pain: low-quality evidence. Only exercise combined with education qualifies (see Moderate).
None
Training
- That pushing and holding produce different long-term benefits. Never tested.
- That pushing suits sustained activation and holding suits rehab and diagnostics. Suggested by researchers, never tested.
- Squeezing with no load (muscle control) as a third kind of isometric. Research recognises only pushing and holding, and studies squeezing only as no-load training.
- The best rest between holds, for muscle or strength. Never studied.
- Isometric holds with weights: very few studies. With bands: none found.
- That 15 seconds is better or worse than any other hold length. Never tested.
- That isometrics relieve tendon pain better than moving exercise. A later meta-analysis says no.
- That isometrics beat lifting overall. They only win on isometric tests.
- That isometrics cause fewer injuries than other training. No evidence either way.
- Anything specific to WITH's format: random 15-second holds spread through a workday. Never studied.
- Pushing on the in-breath, or holding the breath through the whole hold, as some older strength books teach. No supporting evidence, and the breathing findings above contradict both.
- No study has compared daily isometrics with 2 to 3 times a week.
- Isometrics and eating, specifically. No studies found.
- No study has tested warm-up and injury for isometric holds.
Mind
- Serotonin: a 2026 meta-analysis found no effect on blood serotonin.
- Dopamine and oxytocin: no meta-analysis in humans. Most of that evidence comes from rodents.
- Cortisol and stress hormones: only a small correlation, not a proven effect.
- Pain relief from isometrics: two meta-analyses contradict each other. The older one says large; the newer one, using randomised trials only, found none.
Body
- That exercise "boosts metabolism": no meta-analysis found.
- That strength-type breaks protect leg circulation: not significant on their own. The benefit comes mostly from walking and stairs.
Guidelines
- WHO 2020: muscle-strengthening on 2 or more days a week, with no upper limit set. Moderate-certainty evidence.
- WHO 2020: replacing sitting time with activity of any intensity, even light, benefits health. Moderate-certainty evidence.
- ESC 2024: people with high blood pressure should add low-to-moderate isometric or dynamic resistance training 2 to 3 times a week on top of aerobic exercise.
